About Primecare Hospice & Palliative CareAt Primecare Hospice, every moment matters. We are a patient-centered, physician-led, team-based program providing holistic, dignified end-of-life care to patients and families at home, in facilities, and in contracted inpatient settings.Position SummaryThe Admissions Coordinator serves as the central point of coordination for hospice admissions, re-certification visits, and HOPE update assessments. This role ensures compliance with CMS Conditions of Participation, supports accurate and timely documentation in the EHR, and collaborates with the interdisciplinary group (IDG) to establish and maintain individualized plans of care. The Admissions Coordinator also supports staff scheduling, triage coverage, and acts as a resource for patient/family education during admission and re-certification processes.Essential Duties & Responsibilities Admissions & Initial Assessments Complete hospice admission visits in compliance with CMS requirements, including: Initial RN assessment within 48 hours of election. Comprehensive assessment within 5 calendar days. Educate patients and families regarding hospice philosophy, services, and rights. Obtain and document election statement, consents, advance directives, and attending physician choice. Coordinate with IDG to integrate admission findings into the plan of care. HOPE Data Collection Complete HOPE-required assessments as applicable: HOPE-ADM within 5 days of election.HUV1 between days 6–15. HUV2 between days 16–30 (if patient remains active). Document HOPE data accurately in EHR; ensure submission-ready documentation for iQIES. Identify when SFV (Symptom Follow-Up Visit) is triggered by moderate/severe symptoms and coordinate with RN triage/field staff. Recertifications Perform RN recertification visits in compliance with CMS requirements: Understand the face-to-face encounter documentation for 3rd benefit period and beyond. Physician certification/recertification coordination. Update the comprehensive assessment and ensure plan of care reflects recertification findings. Collaborate with IDG during recertification review and care plan updates. Scheduling & Collaboration Assist with staff scheduling as needed or when time permits. Participate in IDG meetings to provide input on admissions, recertifications, and patient status updates. Communicate promptly with the DCS, Medical Director, and scheduling team regarding admission needs and visit coverage. Triage Support Assist with RN triage as needed, including symptom management calls, coordination of urgent visits, and patient/family support. Document triage encounters and escalate to attending physician or IDG as appropriate.
