This role will maintain key strategic partnerships, implement department goals, and supervise staff and contract workers to improve community health outcomes. A key responsibility includes overseeing contracted benefits specialists who assist patients and community members in accessing government assistance programs such as Medicaid, SNAP, and other social service benefits. The responsibilities will also include managing budgets to achieve results.Core Responsibilities And Essential FunctionsProgram Operations & Oversight a. Support implementation and management of Community Health programs and initiatives. b. Oversee contracted benefits specialists, ensuring effective delivery of services that connect patients and community members to government assistance programs. c. Monitor performance and outcomes of contracted services and ensure alignment with strategic goals. d. Collaborate across internal departments and with external partners to enhance program visibility and impact. e. Ability to collaborate and self-start in the areas of program development and implementation, data collection and analysis, and outcomes reporting. Strategic Planning & Partnership Development a. Assist in cultivating strategic partnerships that support departmental goals. b. Contribute to program design and expansion efforts. c. Create and adjust processes to align with strategic plan d. Represent the Community Health department in internal and external meetings. e. Oversee partnership onboarding processes, including contracting. Managing for Results a. Ability to direct and provide support to team members. b. Set clear expectations and holds each team members accountable for actions and results. c. Track processes and outcomes for programs and events. d. Lead, motivate and mentor team members. e. Manage program budgets to align with goals and strategic plan. f. Maintain safety and quality standards established by department and system policies. g. Assist in managing program budgets and tracking expenditures. h. Support data collection, analysis, and reporting for program outcomes and impact. Human Resources Management, Professional Development and Initiative a. Supervise staff, and provide continued support to develop, coach, counsel and mentor. b. Complete mid-year and annual evaluations. c. Coordinate and utilize current reward recognition program for team members. d. Oversee completion of professional competency requirements and education. e. Evaluate and manage employee performance and discipline in accordance with Human Resources Policy and Practice. f. Present creative and new ideas to challenge for efficiencies and quality outcomes. g. Acts independently to lead by example. h. Adapts to the changing healthcare industry and looks to the future for growth and opportunity. Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct.Required Minimum EducationBachelors Health Promotion or Bachelors Business Administration/Management or Bachelors Public Health or Bachelors OtherMasters Public Health or Masters Health Administration or Masters OtherRequired Minimum License(s) And Certification(s)All certifications are required upon hire unless otherwise stated.BLS - Basic Life Support or BLS-I - Basic Life Support - InstructorAdditional License(s) And Certification(s) Required Minimum Experience:Minimum 1 year experience leading community health and social service program development and implementation Required and Minimum 1 year experience leading teams to drive community health and social service improvementRequired Minimum SkillsExcellent interpersonal skills, organizational and communication skills, both oral and written. Excellent time and project management skills. Ability to multi-task with exceptional attention to detail. Ability to critically think and problem solve. Excellent customer service skills.
