Job Description

DescriptionA Qualified Intellectual Disabilities Professional (QIDP) serves as the primary advocate for the individuals and for each individual’s Plan of Care. The QIDP is viewed as crucial in determining the adequacy of an individual’s services and evaluates the individual in all skill development areas to determine strengths as well as programmatic needs. This is an exempt, salaried position. ContributionsMonitors, integrates, and coordinates the development of each individual’s Individual Program Plan (IPP), 90-day Interdisciplinary Team (IDT) Reviews, and IDT meetings. Develops a program for assigned individuals to maximize skill development.

Monitors each individual’s programs and services through direct observation and interaction on a routine basis, noting and following through on any modifications addressed. Describes fully all programs developed for assigned individuals and is responsible for analyzing on a 30-day basis each individual’s Plan of Care noting progress, obtaining data collection, and reviewing all services to ensure the individual is receiving all interventions and/or services identified as needed. Identifies and initiates needs for revision in assigned individuals’ Plans of Care. Prepares necessary programmatic documents and oversees documentation of responsible parties.

Provides ongoing communication with the IDT for each individual to ensure that they are receiving all services as identified in the IPP. Maintains ongoing communication with family members, schools, work programs, and other outside team members. Licensed Doctor of Medicine or Osteopathy.

Educator with an Education Degree from an accredited program. Social Worker with a degree in a field other than social work with at least three years of social work experience under the supervision of a qualified social worker. Physical or Occupational Therapist as defined in Federal Regulations 405. 1101 (m) or (q). Speech Pathologist or Audiologist as defined in Federal Regulations applying to ICFs/MR. Registered Nurse. Therapeutic Recreation Specialist who is a graduate of an accredited program and is licensed or registered in the state. Certified Rehabilitation Counselor.
