As a provider delivering care, support and accommodation for West Australians in greatest need, our purpose is clear, and our passion is strong.Southern Cross Care WA is a place where you can have an extraordinary impact, make an extraordinary difference, and be appreciated for extraordinary contributions.We hope you will join us and become one of the people who make Southern Cross Care WA an extraordinary place to be.About The Role And Where You Will Be WorkingOur Care Coordinators coordinate, guide & oversee service provision (and information) to our Home Care customers.You'll be helping them make decisions, identify and arrange resources that meet their identified care needs.We'll also be asking you to oversee day to day management of funds / budgets.This is a mobile and office-based role, with the majority of your time spent in the community visiting clients in your care portfolio.Please note there are two vacancies; one to cover North of the River & one to cover South of the River.We’ll Also Be Asking You ToProviding great customer service & advocating for your clientsUndertaking comprehensive assessments of identified needsDeveloping goal & care plansprovide guidance to promote enablement through informed decision making and workable solutionsCompleting all required documentationIdentifying & responding to customer's changing needsWorking collaboratively with clinical & allied health servicesAbout YouWith a well-developed knowledge of the aged care sector, you have experience in a coordination / case management role.As a natural problem solver, you can work autonomously and also work effectively as part of a multidisciplinary team.You're comfortable managing complex clients who are receiving Home Care packages.To Be Considered For This Role, You’ll NeedExperience in aged care or community services.Familiarity with home care packages.Great interpersonal & communication skills.Ability to quickly learn & use new software for record keeping.Working With UsAt Southern Cross Care WA, we are committed to being an employer of choice.
