What You'll DoThis role balances hands-on case management with team leadership and supervision. You'll help shape the service delivery strategy while ensuring consistent, high-quality support for clients across outreach programs.
Your Work Will IncludeProviding direct case management services to clients and supporting staff follow-up on referrals. Supervising a team of Outreach & Health Ministry case managers, including onboarding, coaching, scheduling, and conducting performance evaluations. Ensuring compliance with internal and external documentation standards using EHR, HMIS, and ETO systems. Building and maintaining relationships with external referral partners in housing, health, mental health, and addiction services. Participating in staff and agency meetings, trainings, community needs assessments, and public-facing events. Supporting program logistics, including volunteer engagement, team planning, and coordination with outreach schedules.
Driving outreach vehicles as needed and maintaining a visible presence in field-based work. What We're Looking ForYou're a skilled case manager, with experience in Chicago housing case management, and thoughtful team leader who brings cultural humility, clarity, and consistency to your work. You're able to respond calmly in challenging situations and guide others with integrity and compassion. QualificationsBachelor's degree in social services or a related field. 2–5 years of experience in housing-focused case management. Experience supervising or mentoring staff in direct service settings. Valid driver's license, clean driving record, and access to a vehicle. .
Ability to work outdoors and during varied hours (day, evening, weekends). Strong communication and documentation skills. Familiarity with harm reduction, trauma-informed care, and culturally competent practices. Bonus if you: Are bilingual in Spanish and English.
