Apply today!POSITION: Clinical Manager, Pacifica House - Full-Time (Monday - Friday, 8: 30am - 5: 00pm)Position SummaryThis position will be responsible for planning, initial implementation of all aspects of patient care and client satisfaction as it relates to the Hospice House interdisciplinary team. This position is responsible for ensuring that the PH team are in full compliance of the POC for each individual patient.
The Clinical Manager is an experienced registered nurse who follows the hospice COP's while admitting patients to PH on GIP, Respite, and Routine level of care, and coordinates the services provided to his/her patients. The Clinical Manager works cooperatively with the attending physician, Hospice House Medical Director, and other members of the interdisciplinary Hospice team. The Manager participates in patient care conferences and supervises the RN's, Home Health Aides, LVN, and housekeeper.
The Manager provides oversight to the care given to patients who have been diagnosed with a terminal illness and regularly reviews and ensures the following of the plan of care. The Manager ensures that all prescribed medical treatments, pain management and symptom control, assessments and evaluations are completed and documented according to the HNC Model of care. This position provides education and supportive care to patient and caregiver. The Manager is responsible for staffing, ordering of house supplies, and provides oversight to all of the outside contract staff who utilize PH.
Responsible for ensuring all of The Joint Commission and State regulations are adhered to. QualificationsEducation: Graduate of an accredited school of nursing with baccalaureate or higher degree in Nursing or another health-related field. CredentialsCurrent CA RN License. BSN preferred. Certification in Hospice and Palliative Nursing (CHPN) desired. ExperienceThree years of experience within the last five years in a hospice or home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity.

Core CompetenciesDemonstrated competency in performance of professional nursing functions. Requires excellent oral and written communication skills. Ability to work with dying patients and their families. Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the care team.
Has the ability to assess the quality of care that is provided by the team members at PH. Must be able to work with the management team at HNC and support the policies and procedures that are in place for PH. Adheres to all CMS and state guidelines and nursing standards of practice.
