The post holder will coordinate and deliver safe, effective, and evidence-based insulin pump initiation, education, and ongoing management, empowering patients to achieve optimal glycaemic control and improved quality of life.Must have own transport and be able to travel ( home Visits ) and work across sites and cross hospital group settings .Closing date10/11/2025 10:00:00Proposed interview dateInterviews will be held as soon as possible after the closing date.
