There is a place for you at Highland Rivers Behavioral Health!Highland Rivers Behavioral Health provides treatment and recovery services to individuals impacted by mental illness, substance use disorders and intellectual developmental disabilities.The Crisis Respite Coordinator supports individuals transitioning from statehospitals, incarceration, and/or homelessness by teaching them coping skills and independentliving skills that are necessary for recovery. The Crisis Respite Coordinator assists in completing ahousing support plan, to include crisis resources and wellness tools that will empower theindividual served to live independently in the community of their choosing. The Crisis RespiteCoordinator connects individuals served to needed resources, including the Needs for SupportiveHousing Survey.Job Duties Include, But Are Not Limited ToComplete Weekly GRID updates and Continued Stay Criteria Docs sent to Utilization ManagementComplete CONNECTS and Tx Plans, crisis planning, housing support plansTransports INDs to program-related appointments as neededAssists INDs in applying for/obtaining vital records, food stamps, other necessary resourcesSubmits referrals to appropriate services (SOAR, Supported employment, case management, MD appointments, etc)Assists IND in applying for housing voucher or permanent supportive housing when appropriateReview ECRA referrals and enter into ECRA Referral SpreadsheetECRA Health and Safety Logs (For H&S Officer only)Document face-to-face and/or over-the-phone sessions daily in ECR; including complaints, results of assessments, etc. Maintain accurate and detailed participant recordsComplete HQS inspections for unitsOther duties as assigned.
