Using the YWHO intake processes and standardized assessment tools, this role gathers relevant information to determine service needs, supports measurement-based care, and facilitates warm transfers to Hub programs, partner agencies, and external service providers.The Intake Coordinator/Care Navigator works closely with youth to support engagement and continuity of care, liaises between Hub staff and community partners, and contributes to trauma-responsive, culturally safer, and youth-centered service delivery.Key ResponsibilitiesIntake & AssessmentConduct intake interviews with youth using YWHO standardized forms and proceduresCollect information to determine appropriate service pathwaysExplain measurement-based care to youth and support them in completing assessmentsEnsure intake processes are trauma-responsive, culturally safer, and youth-centeredCare Navigation & Follow-upFacilitate warm transfers to Hub programs, partner agencies, or external service providersSupport youth in maintaining engagement and continuity of careTrack follow-ups and outcomes, advocating for youth as neededLiaise between youth, Hub staff, and external providers to ensure coordinated carePartnerships & Network EngagementBuild and maintain strong relationships with schools, community agencies, and youth-serving networksParticipate in collaborative initiatives and community tables to strengthen the Hub's referral and support networkSupport identification of service gaps and opportunities for improved youth accessDocumentation, Data & Quality ImprovementMaintain accurate, confidential records of intake, assessments, referrals, and follow-upsParticipate in data collection, reporting, and continuous quality improvement activitiesMonitor key performance indicators (KPIs) aligned with Hub, funder, and YWHO expectationsCollaboration & SafetyWork collaboratively with Hub staff, including CYW/Facilitator, Youth Outreach Worker, Peer Support Worker, to ensure integrated, youth-centered careAdhere to health, safety, and crisis response protocolsQualificationsDiploma or degree in Social Work, Social Service Work, Child and Youth Care, or related fieldKnowledge of youth mental health, substance use, wellness services, and community resourcesMinimum 2 years experience with care navigation, intake, or assessment processes, preferably in a youth-serving contextUnderstanding of trauma-responsive, culturally safer, and anti-oppressive approachesExcellent communication, relationship-building, and advocacy skills, with ability to engage youth and familiesAbility to build and maintain collaborative relationships with external partners and service providersFamiliarity with measurement-based care, YWHO intake tools, and standardized assessment forms is an assetProficiency with data entry, reporting, and quality improvement toolsValid Ontario driver's license and unlimited use of a vehicle is a requirementFlexibility to work some evenings and weekends to support youth accessSatisfactory criminal reference and vulnerable persons checkFull vaccination against COVID-19 is mandatory for this position (DCHC will however adhere to its duty to accommodate those who are unable to be fully vaccinated for a reason related to a human right protected ground).Application Process:While we thank all applicants for their interest in applying, only those qualified and considered for an interview will be contacted.
