We are the only provider in Australasia to have attained CARF Accreditation, an international gold standard for person centered quality rehabilitation. We have a mix of allied health professionals, psychologists and medical professionals that work across our paediatric, adult and Māori health teams.A full-time, permanent position has become available to join our friendly, innovative inter-professional IDT team. The role of the keyworker is unique to our organization. It is a key link between our Kiritaki / whānau and the ABI team.This role will involve providing keyworker services in our intensive in-patient service. We are a 36 bedded inpatient facility with length of stay between 2 weeks to a maximum of 6 months.The applicant must have excellent communication skills, establishing relationships with our stakeholders, strong teamworking and organization/ planning skills.The Case Load Is Varied And Key Areas IncludePromoting a smooth and efficient pathway for clients and families-whānau from admission through to discharge.Communication throughout rehabilitation journey with clients, whanau, ACC, DHB, rehabilitation providers and external organizations.Ensuring that families-whānau are engaged in the rehabilitation process as an integral part of the rehabilitation team.Proactively managing discharge planning processes in consultation with the IDT.Monitoring family-whānau satisfaction and promoting quality improvements.Developing and implementing family-whānau information and education systems.Commitment to the principles of Te Tiriti o Waitangi.We can offer you excellent opportunities for professional development including formal supervision, regular in-service training and attendance at external workshops and conferences to support professional learning.A minimum of two (2) years’ experience working with clients in similar rehabilitation settings would be preferred.
