Licensed Social Worker

We are seeking a full-time Social Worker (CSW / LCSW) to support social service programs in a healthcare facility.Assist with planning, developing, and evaluating social services and community resources.Support residents and families through care coordination, discharge planning, and assessments.Coordinate transportation and refer residents to community agencies.Participate in community planning related to resident needs.Engage residents and families in program planning when appropriate.Requirements: Bachelor’s degree in Social Work or related field, minimum 2 years experience in a nursing facility, active CSW or LCSW license, strong communication skills.Benefits include: Competitive pay, 401(k), health/dental/vision insurance, paid time off.Location: Montgomery, PA (must relocate before starting).This role involves in-person work.
