In this unique hybrid role, you will walk alongside individuals navigating mental health treatment, helping them connect to essential services both within Pawnee and throughout the community.
From guiding clients through their intake to advocating for financial or social benefits, you serve as both a care coordinator and a resource navigator. This role plays a vital part in breaking down barriers to treatment, ensuring clients receive the care, support, and follow-up they need to succeed.
The Day-to-DayGuide clients through the intake process by completing vitals, screenings, and connecting them to appropriate medical or community resourcesServe as an ongoing care coordinator by supporting clients throughout treatment, performing follow-up calls, and helping them stay connected to servicesAssist clients in navigating and applying for benefits such as Medicaid, Medicare, SSI/SSDI (including SOAR), and other financial supportsCollaborate with internal teams, government agencies, and community partners to advocate for client needs, resolve barriers, and coordinate care transitionsMaintain accurate documentation, meet care coordination expectations, and travel as needed across our 10-county service areaWhy Work With Us?At Pawnee Mental Health, we take a great deal of pride in the services we offer, and the progress made by those we serve.

RequiredRequires a Bachelor’s degree in a human services field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education. Requires an extensive knowledge of SSI/SSDI and other public entitlement programs. Basic personal computer skills which enable the entry, retrieval and use of electronic data required; Strong oral and written communication skills using the English language required. A valid driver’s license and automobile liability insurance maintained in accordance with Pawnee policy and procedure is required.
All employees will be required to submit to and pass a background check and drug screening. PreferredExperience with and knowledge of the needs and challenges of clients who experience mental illness and/or chemical dependency preferred. Experience with Microsoft Office software preferred. Salary Description$24. 04/hour
