Job Title: Planning List Navigator- Region 3 Field Office- AtlantaPay Grade: IWorkplace: Hybrid- 2 days a week per officeTravel: NoneJob SummaryThe candidate chosen for this position will perform the duties listed below in multiple counties in Georgia.Must be a highly organized individual, as this position will require a significant amount of tracking and follow-up for many different job functionsRequires completion of a significant number of Needs Assessments for new individuals as well as annual renewals for individuals on the Planning ListRequires telephone support to individuals and families on the Planning List, and bringing resolution to phone calls and messagesRequires reporting Planning List changes and concerns to Planning List SupervisorMake and resolve referrals to outside agencies for non-waiver servicesOther supports for Planning List as needed.Preferred Qualifications - ExternalExperience with or knowledge of individuals with intellectual/developmental disabilities, Medicaid waivers, and DBHDD I/DD services; experience working successfully without daily contact with a supervisor.Spanish fluencyFor benefits information, please visit: https://careers.dbhdd.ga.gov/why-dbhdd/Employment RequirementsTo ensure the safety and wellbeing of our employees, individuals and communities we serve, certain positions within DBHDD may require evaluations, vaccinations, professional licensure and certifications.
