Planning, coordination and delivery of quality medical social work services, assuring compliance with corporate standards and federal, state and local regulationsProvides ongoing assessment of clients, assuring an accurate evaluation of client needsDevelops a comprehensive care plan that addresses the social work needsWorks with patients and families regarding coping with illness, accessing resources, or other issues affecting the patient's health status Links client/family with needed servicesWhat do I need for this role with Redstone?Bachelors in Social Work. Two (or more) years of related experience in a health care setting (preferred).
One year of experience in home health/hospice (preferred). Must possess valid driver's license.
