About the Position:The RN Care Coordinator position plays a critical role in managing patient care in our ambulatory clinics, with focused efforts on the management of diabetes and hypertension health conditions. This role involves patient education, care plan development, and patient advocacy by coordination of care and services. The role is primarily Mondays through Fridays, 8 am-5 pm, with no weekends or holiday requirements. $5,000 Sign On Bonus — paid in two incrementsCompensation & BenefitsHourly Rate: $35.50 – $37.00 per hour (based on experience)Generous Paid Time Off to recharge and spend time with loved onesBirthday Holiday plus 7 additional paid holidays each yearMedical, Dental & Vision Coverage to keep you and your family healthyCompany-Paid Life Insurance for peace of mindRetirement Plan with Company Match to help you plan for the futureEmployee Assistance Program for confidential support and resourcesWhat You’ll DoCoordinate care for patients with diabetes, hypertension, and other chronic conditions Provide medical education for patients and families Support programs including Hepatitis B and Lead Prevention Manage nurse scheduling and perform triage calls Handle STAT referrals, specialty referral follow-ups, and patient advocacy Oversee Synagis and RSV vaccine administration and education Collaborate with a multidisciplinary team to improve patient care and access Care Coordination & ManagementLead and collaborate with the patient-centered care team to ensure top-quality, preventive-focused careSupervise a care coordination team (Patient Navigator, Unit Clerk, Medline Assistant)Support high-risk patients through assessments, care planning, and, when needed, home or hospital visitsEducate patients, families, and caregivers on the Patient-Centered Care ModelService Coordination & AdvocacyProvide case management, client advocacy, and goal-based care plansBuild relationships with patients and families to encourage treatment adherence and self-managementServe as the go-to resource for patients regarding medical and dental benefitsMonitoring & ReportingTrack and document patient progress, addressing barriers to careMaintain strict HIPAA compliance and patient confidentialityCollect and analyze data to improve services and prepare monthly reportsOther ResponsibilitiesTake on additional duties that support patient care and clinic successWhat We’re Looking ForExperience: At least 2 years in community health, public health, outreach, social work, or related fields — with progressively responsible leadership experience.Specialty Knowledge: Pediatric experience required.Leadership: Background in care coordination or care management in a clinical setting.Technical Skills: Comfortable with data entry and EHR experience. People Skills: Exceptional communication and interpersonal abilities to connect with individuals from diverse backgrounds.Team Mindset: Ability to collaborate with clinic management, providers, and staff for seamless patient care.Licensing: Graduate of an accredited RN program, with a current license to practice in Pennsylvania.Preferred QualificationsFamiliarity with The Joint Commission regulationsBilingual abilities in Spanish, Nepali, or FrenchHamilton Health Center (Hamilton), established in 1969, is a Federally Qualified Health Center (FQHC) located in Harrisburg, PA.
