DEPARTMENT: SWM GERO PSYCH UNITFACILITY: Southwell MedicalWORK TYPE: Full TimeSHIFT: DaytimeSummaryThe Social Services Coordinator provides for social, emotional, and psychological needs of patients and promotes continuing growth and development to the individuals' full potential.ResponsibilitiesEnsures the Psycho-social evaluations are completed in their entirety and in a timely fashion (within 72 hours of admission).Group, individual, and family sessions are conducted and completed according to group agenda, per M.D. orders on appropriate patients as necessary.Assist in addressing patient rights, grievances, monitoring patient/family experiences.Completes Inquiries and Determinations of Treatments in a timely manner and passes report on any anticipated admissions to the charge nurse and program manager.At the time of discharge, ensures social services coordinates or supervises the coordination of all anticipated needs of the patient in a timely manner, as evidenced by documentation.Completes monthly audits of social services in compliance with CMS and Joint Commission.Completes all state paperwork and evaluations to facilitate patients being placed into nursing homes, assisted living facilities, or personal care homes.Serves as primary case manager for patients on the unit.Assists in patient/family orientation to unit.Actively participates in treatment team and is responsible for treatment plan documentation.Encourages family participation in the plan of care to maximize rehabilitation potential.Ensures patient and/or family signatures are on treatment care plans.Documents in medical record and treatment care plan as required.Coordinates unit educational activities with Program and Nurse Manager.Facilitates the placement of patients in long term care facilities as needed/appropriate.Provides assistance with completion and coordination of referrals including support with the referral line and communicating with referral sources and patients when needed to assist with admission process.Keeps abreast of all pertinent federal, state and facility regulations, laws and policies as they presently exist and as they change or are modified..Demonstrates age-specific skills as required by position.Offers suggestions on ways to improve operations of department and reduce costs.Attends all mandatory education programs.Improves self-knowledge through voluntarily attending continuing education/certification classes.Maintains required competency levels as identified in written exams, skills checklists, skills labs, annual safety and health requirements as well as service excellence education hours requirements.Willing to cross-train in order to better assist co-workers and to provide maximum efficiency in department.Volunteers/participates on hospital committees, functions, and department projects.Manages resources effectively.Reports equipment in need of repair in order to extend life of equipment.Makes good use of time so as to not create needless overtime.EDUCATIONBachelor's Degree in Psychology preferredCREDENTIALSOther InformationThree (3) years of experience working with geriatric population, preferred.Southwell/Tift Regional Health System, Inc. is an Equal Opportunity Employer.
