Your Mission: Upon referral, assesses health and psychological needs and coordinate care services for Independent Living residents. Develops individual wellness plans/interventions for residents and oversees implementation of plans. Establish linkages with agencies and service providers and monitor the services being administered to residents. Provides education and support to residents as needed.

Key ResponsibilitiesObtain and review documentation from prospective service providers to ensure compliance with Sun Health At Home standards.
Schedule and conduct pre-admission assessments for prospective members, including review and analysis of medical records. Present findings to the Executive Director and Care Coordination Committee. Conduct welcome meetings for new members, ensuring a smooth and efficient onboarding process. Maintain regular communication with members, including at least one in-person visit per year, quarterly phone calls, and additional contact as needed. Perform annual and as-needed assessments to evaluate members' needs and determine appropriate services. Visit members in hospitals or care centers, virtually, or at home to assess needs and provide reassurance and support.
Coordinate services that align with each member's individualized wellness plan and membership coverage. Ensure services are medically appropriate and compliant with program guidelines. Clearly communicate covered services and any out-of-pocket costs to members. Exercise sound financial judgment by managing care delivery within departmental budget guidelines, ensuring services are cost-effective and in accordance with SHAH membership agreements. Promote prevention and early intervention by developing and delivering proactive wellness services that support members in maintaining independence, avoiding health crises, and aging successfully at home. In partnership with the Executive Director, lead quality monitoring efforts and serve on the Quality Monitoring Committee.
Participate in member recruitment activities, including informational seminars and presentations to prospective members. Foster an environment that supports member privacy, dignity, and well-being in a safe and secure setting. Maintain a customer-service-centered approach in all member interactions. Document all member interactions, care coordination efforts, and follow-ups using the electronic medical record system. Demonstrate the Sun Health Mission and Values. Your QualificationsBachelor's or master's degree in nursing, gerontology, social work or occupational therapy with related experience preferred.
A minimum one year of experience working with older adults and understands the challenges of aging in place in one's home in the community. A minimum one year of experience in compliance with patient privacy and HIPAA regulations and requirements. Must be proficient with MS Office and email software and electronic medical record charting. Possess current or has the ability to obtain an Arizona Identity Verified Prints (IVP) Fingerprint Clearance Card is required.
