Job Description

As a Support Coordinator with Solve Care, you'll engage with NDIS participants to guide and coordinate implementation of their plans, connect them with services and supports, monitor progress, and build their capacity to achieve meaningful outcomes. You'll operate as your own business (contractor) while working alongside a values-driven provider committed to excellence.Applicants must be Australian citizens or permanent residents.A minimum of 1-2 years of NDIS experience is required.Candidates must hold a current NDIS Worker Check.Liaise with participants, families/informal supports, funded and mainstream providers to ensure effective plan implementation and coordination of supports.Assist participants to understand their NDIS plan (support categories, budgets, provider options) and make informed choices.Develop and maintain relationships with service providers and stakeholders; ensure timely delivery of supports aligned with participant goals.Monitor participant progress, track budgets, report on outcomes, identify risks, and support plan reviews.Facilitate capacity building so participants gain more independence, self-advocacy, and choice in managing their supports.Maintain detailed, compliant documentation and case-notes consistent with NDIS Practice Standards and Solve Care's policies.Operate in line with contractor business expectations.Applicants must have an existing ABN for contractor business operations.Excellent organisational, time-management and documentation skills.Demonstrated experience in a Support Coordination role, or equivalent case-management/co-ordination experience in the disability/NDIS sector.Sound understanding of the NDIS environment: plan implementation, support coordination, mainstream/informal supports, capacity building.
