The Veteran Healthcare Navigator position plays a vital role in coordinating with other healthcare systems ensuring appropriate care coordination for veterans participating in the SSVF program. In this role, the Veteran Healthcare Navigator will serve veteran families from Jackson to the MS Gulf Coast, with families from the Pine Belt Mental Health service area being the primary focus.For 50 years, Pine Belt Mental Healthcare Resources has served individuals throughout the Pine Belt. Through an extensive continuum of care, we strive to enable adults with serious mental illnesses, children with serious emotional disturbances, and individuals with an addiction or developmental disability to live, work, learn, and participate fully in their communities. Our eighteen-county service area includes Amite, Covington, Forrest, Franklin, Greene, Hancock, Harrison, Jeff Davis, Jones, Lamar, Lawrence, Marion, Pear River, Perry, Pike, Stone, Walthall and WayneRequirementsMaster's degree in behavioral healthcare or related field preferredLCSW or equivalent background preferredMust have reliable transportation to provide community servicesMust have a valid driver's license with a satisfactory driving recordExcellent Communications Skills RequiredIndependent work abilityVeterans encouraged to applyResponsibilitiesMaintains and updates contact list of VA and other community programs.Maintains successful working relationship between SSVF program and providers.Identifies and attempts to remove barriers to care for veteran participants.Assists veterans in gaining access to healthcare, supporting healthcare plans by identifying barriers to care, and providing education on wellness-related topics.
