Role DescriptionThis is a full-time on-site role for the Director of Social Services, located in Encinitas, CA. The Director of Social Services will be responsible for overseeing the social services department, coordinating care plans, conducting social work, managing discharge planning, and providing case management.
The Director will work closely with residents, families, and healthcare staff to ensure a comprehensive approach to social services and care management within the facility. QualificationsRequired: Master's degree in Social Work or Bachelors in Social Work with 1+ years experience in healthcare settingExperience in Skilled Nursing preferredExperience in Care Planning and Social WorkSkills in Discharge Planning and Social ServicesProficiency in Case ManagementExcellent interpersonal and communication skillsAbility to work collaboratively with diverse teams
