Role Description The Clinical Care Partner is a full-time, on-site role based in Queensland, Australia. This role involves collaborating with individuals and their support networks to develop and implement care plans tailored to their unique needs. Responsibilities include providing hands-on care, coordinating multidisciplinary services, monitoring client progress, and ensuring a high standard of support. The Clinical Care Partner will also ensure compliance with care standards and conduct regular assessments to adapt plans as needed. Qualifications Skills in care planning, client assessment, and hands-on support deliveryStrong communication and collaboration abilities with clients, families, and stakeholdersKnowledge of NDIS practices, aged care standards, or community health principlesAdaptability to provide culturally appropriate and individualized careExperience with documenting care plans, reporting, and maintaining compliance recordsRelevant qualifications in Nursing, Allied Health, or Community ServicesCurrent First Aid and CPR certification, Blue Card, and NDIS Worker Screening ChecksPrevious experience in disability, aged care, or in-home support is highly desirable
