About The RoleWe are seeking two experienced and dedicated CHC Specialist Practitioners to join our Adult Social Care team. These roles are pivotal in ensuring that individuals with complex health needs receive the appropriate care and support through the NHS Continuing Healthcare (CHC) and joint funding processes.Key ResponsibilitiesRepresent Adult Social Care in Decision Support Tool (DST) Multi-Disciplinary Team (MDT) meetings and joint funding arrangements.Contribute to CHC assessments, reviews, and appeals.Collaborate effectively with health and social care professionals to ensure integrated care planning.Provide expert advice and guidance on the CHC National Framework and joint funding arrangements.Maintain accurate and timely records in line with statutory and local requirements.Advocate for individuals with complex health needs, ensuring their voices are heard and their rights upheld.Essential CriteriaRegistered practitioner: Social Worker, Nurse, or Occupational Therapist.Proven experience in CHC assessments or participation in DST MDTs and joint funding processes.Strong understanding of the CHC National Framework and complex health needs.Excellent communication and negotiation skills.Ability to work collaboratively within multi-disciplinary teams.Commitment to person-centred care and safeguarding principles.About The TeamYou will join an energetic and supportive team striving to support people in Southampton live active, safe and independent lives. The Adult Social Care CHC Team represents Southampton City Council Adult Social Care at Decision Support Tool (DST) Multi-Disciplinary Team (MDT) meetings to determine NHS Continuing Healthcare (CHC) eligibility. The team works collaboratively with health partners to agree joint funding arrangements, including Section 117 aftercare (in liaison with relevant adult social care teams), and provides expert advice to social care teams on complex health and care needs.
