And we are scaling that impact across the state.If you are a mission-driven clinician who wants to transform care at the community level, we want you on our team.About The RoleThe ECM Clinical Supervisor (Clinical Consultant – Enhanced Care Management) provides clinical oversight, supervision, quality assurance, and CalAIM-compliant care review for APF’s Enhanced Care Management (ECM) program.This Clinician Ensures That Services Are clinically appropriate culturally and linguistically responsive compliant with MCP (Managed Care Plan) standards trauma-informed, whole-person, and equity-centeredThis is a flexible part-time contractor role (~32 hours/month) designed to support up to 125 ECM clients across APHealth ECM teams.What You'll DoClinical Oversight & SupervisionProvide routine clinical supervision to ECM Care Managers & CHWsReview & sign all ECM care plans (initial + annual updates)Conduct monthly chart reviews for MCP and CalAIM complianceGuide staff on risk mitigation, crisis intervention, and escalation pathwaysSupport complex case consultationsQuality Assurance & ComplianceLead continuous quality improvement and documentation auditsSupport MCP audits and quality reviewsEnsure all services meet ECM contract and state guidelinesDeliver ongoing clinical training and upskillingPartnerships & RepresentationRepresent APF in MCP meetings and clinical contract reviewsBuild collaborative relationships with hospitals, FQHCs, BH providers, and housing partnersDirect Service Support (as needed)Provide short-term assessments or case conferencing during high-volume periodsHours & Compensation~32 hours/month (approx. 8 hours/week)Base stipend: $2,000/monthPlus $50 per care plan review and signatureEstimated annual compensation: ~$30,250 (equivalent to ~$79/hour)A highly flexible schedule is available.QualificationsBilingual (Spanish/English or other threshold language)Experience with CalAIM ECM and/or Community SupportsPrior supervision of interdisciplinary or care management teamsWhat Success Looks Like100% timely review & signature of ECM care plans≥90% documentation and audit complianceStrong collaboration between ECM teams, CHWs, and MCP partnersEffective management of high-risk and escalated casesA well-trained, culturally responsive care teamWhy Join APF?Because this is bigger than a job.
