Apply today!POSITION: Hospice Clinical Liaison - Full-Time (Monday - Friday, 8: 30am - 5: 00pm)Position SummaryThe Hospice Clinical Liaison serves as a vital link between Hospice of the North Coast (HNC) and the community, blending clinical expertise with outreach and relationship-building skills. This role is ideal for a licensed RN, LVN, LCSW, or MSW who brings both healthcare experience and a passion for connecting with others. The Clinical Liaison is responsible for increasing hospice admissions through strategic referral development, education, and community engagement.
With a strong understanding of hospice and palliative care, the Liaison collaborates closely with HNC's interdisciplinary team to ensure timely, compassionate care while serving as a trusted clinical resource to patients, families, and providers.
