We’re looking for Care Coordinators (CTC) to join our growing team and play a key role in supporting the onboarding of patients into our Chronic Care Management (CCM) program.About The RoleAs a CTC, you’ll be responsible for educating patients on CCM services, obtaining consent, creating the first care plan, and ensuring a smooth relationship with your patients as you help then navigate through CCM. This is an excellent opportunity for someone who thrives in a collaborative, patient-centered environment and enjoys a mix of communication and administrative responsibilities.What You’ll DoConduct initial outreach calls to introduce the CCM program and obtain patient consentDevelop and document the first care plan in alignment with CCM requirementsComplete the initial CCM reportEnsure all documentation is accurate, timely, and compliant with regulatory standardsDevelop medical relationships with PatientsAddress any questions or concerns patients may have during the onboarding processSupport various administrative tasks related to enrollment and care coordinationWhat We’re Looking ForStrong communication skills with a patient-first approachDetail-oriented with excellent documentation and follow-up habitsAbility to work independently while contributing to a collaborative teamExperience in healthcare, care coordination, or customer service preferredScheduleFull-TimeMonday–Friday8 hour day- open to 4-10s SchedulingBenefitsEligibility for Benefits: Begins on the 1st of the month following 60 days of employmentHealth Insurance: 50% premium paid for employee coverage; family plans available at competitive ratesDental & Vision Insurance: Offered through our group planPaid Time Off (PTO): Accrues up to 40 hours per year; accrual begins on day one, available after 90 daysUnpaid Time Off: 40 hours per year, available after an initial training periodPaid Holidays: Six (6) paid holidays per year after 90 days: New Year’s Day, Memorial Day, Independence Day, Thanksgiving, Christmas Eve, and Christmas DayCompensation:Compensation features supplemental pay: earn per patient served with a guaranteed minimum hourly wage of $16–$17, based on licensure and experience.Powered by JazzHRXO2ZAO4L8I
