Job Description

RegularNon-ExemptGENERAL DESCRIPTION:Oversees interventions and/or calls to clients (beneficiaries, providers, pharmacies) to promote the correct use of medications and follow the prescription to increase adherence in the identified population, which results in a significant impact on the star rating for the company. Participates in the management of clinical interventions with beneficiaries/providers, offering recommendations for the use of statins in patients with diabetes (SUPD), cardiovascular disease (SPC), concurrent use of opioids and benzodiazepine (COB) and/or polypharmacy’s measures (ACH, CNS). Receivers and administers referrals assigned to the pharmacy department to address service situations related to drug coverage for both business lines.ESSENTIAL FUNCTIONS: Performs clinical interventions for clients (beneficiaries, providers, pharmacies, and doctors) related to medications, where beneficiaries' adherence impacts the star rating. Educates and promotes patients so that they can obtain their prescribed medications by the clinical and administrative criteria established by the pharmacy department. Identifies patients who are not adhering to medication therapy (quality measures: diabetes, hypertension, cholesterol) intending to proactively impact the star rating for the company. Participates in clinical interventions management with beneficiaries/providers in offering recommendations for the use of statin in patients with diabetes (SUPD), cardiovascular disease (SPC), concurrent use of opioids and benzodiazepine (COB) and / or polypharmacy’s measures (ACH, CNS). Administers and documents clinical interventions in the Rx Effects and Aspect VIA tool. Participates in initiatives determined by the pharmacy department or by the company that may impact on the star rating, such as educational talks or clinical activities with beneficiaries or their physicians to improve adherence inside or outside the company. Must comply fully and consistently with all company policies and procedures, with local and federal laws as well as with the regulations applicable to our Industry, to maintain appropriate business and employment practices. May carry out other duties and responsibilities as assigned, according to the requirements of education and experience contained in this document. MINIMUM QUALIFICATIONS:Education and Experience: Pharmacy Assistant Technical Course or associate degree. At least one (1) year of experience providing service in the community or institutional pharmacies (hospitals, diagnostic and treatment centers (CDT), nursing services facilities, or asylums).Certifications / Licenses: A valid Pharmacy Technician License and Registration is required.Other: Knowledge of ICD-10 clinical criteria and/or CPT procedure codes.Languages:Spanish – Advanced (comprehensive, writing and verbal)English – Intermediate (comprehensive, writing and verbal)“Somos un patrono con igualdad de oportunidad en el empleo y tomamos Acción Afirmativa para reclutar a Mujeres, Minorías, Veteranos Protegidos y Personas con Impedimento”
