Job Description

Job SummaryServe as the Population Health Manager (PHM) providing overall planning, direction, coordination, outreach, management, and supervision for the population health of Hospital Authority of Miller County (HAMC). The PHM is responsible for leading, coaching, and developing the Care Management and Outreach Teams. The PHM will work closely with leaders/organizers to service existing clients within the organization and community to partner in care and treatment and establish up and midstream interventions that affect future patient outcomes. The PHM will be responsible for driving the overall performance management and compliance that contribute to the goals and objectives of the Rural Health Clinic (RHC).General RequirementsPerforms all job responsibilities in alignment with the mission and vision of the organization. Performs other duties as required and completes all job functions as per departmental policies and procedures. Maintains current knowledge in present areas of responsibility (i.e., self-education, attends ongoing educational programs). Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time. Wears protective clothing and equipment as appropriate. General SkillsAbility to communicate in English, both verbally and in writing. Additional languages preferred. Strong written and verbal skills.Basic Computer SkillsWorking ConditionsGeneral environment: Works in a well-lighted, air-conditioned area, with moderate noise levels. May be exposed to high noise levels and bright lights. May be exposed to limited hazardous substances or body fluids, or infectious organisms. May be required to change from one task to another or different nature without loss of efficiency or composure. Periods of high stress and fluctuating workloads may occur. May be scheduled as needed including overtime. Physical Requirments & DemandsHave near normal hearing: Hear alarms/telephone/normal speaking voice. Mission StatementQUALITY HEALTHCARE: In our continuing effort to enhance the quality of life for the communities we serve, the Hospital Authority of Miller County is committed to the delivery of superior, safe, cost-effective healthcare through the provisions of education prevention, diagnosis and treatment.Job Specific CompetenciesOversees Performance Goals and Quality Measures within the RHC, Employee Wellness, Specialty Clinic, outreach programs, and expanded activities and evaluate effectiveness while aligning strategic planning and objectives for future population health program success.Effectively identifies and implements strategies and timelines for performance improvement.Lead, coach and develop a high functioning field-based clinical outreach team.Manages daily activities in affiliated clinics as it pertains to quality measures and reporting.Participates in scheduled RHC, specialty clinic, outreach programs and organization meetings while utilizing these meetings as a forum to collaborate in efforts to promote success of the population health program. Prepares quarterly action plans to establish goal centered measures to ensure up to date quality reporting. Works with coders and billers to ensure correct documentation is being sent to payers for quality reporting.Participate in facility led education and skill competencies for professional development. Effectively handles all personnel issues as it relates to the care coordination/outreach team in regards to the affiliated practices.Mentors practice managers by providing leadership, positive reinforcement to facilitate growth and practice operations.Work with Physician Referral / Discharge Planning team members and Clinical Outreach representatives to negotiate and secure partnerships.Fosters relationships within the organization to embrace Miller County Hospital’s mission and vision while expanding intervention as that contribute to effective transitional care and population health.Promotes a culture of service excellence while utilizing critical thinking, innovativeness, and teamwork.Present data and education to community stakeholders to increase disease management awareness. Attend and participate in select conferences where opportunity exists to market HAMC’s Healthcare’s services.Participate in business development, team meetings and assigned projects.Remain current on healthcare industry developments/trends and assigned states regulations.Ensure all administrative duties are complete, timely and documented; including managing team, activities, performance and communication.Monitors and analyzes medical management activities; provides support to clinical programs; may perform clinical assessments, biometric screenings and clinical audits as needed.Develops and implements strategies that promote local health education resources and address identified member health education needs.Will establish workflows and processes that effectively promote RHC and specialty clinic goal achievement being mindful of needed change that supports evolving goals, updates in the EMR, fluctuations in staffing and staff responsibilities, etc.Establish positive relationships with community-based organizations and other key partners to gain understanding of the cultural, social, and linguistic needs of members.Develops and implements strategies that promote population health education resources and address identified member health education needs.Provide training for office staff on a timely and as-needed basis.Complies with organizational policies and procedures.Adheres to and practices the standards of performance outlines in the Standards and Performance handbook. Duties, responsibilities and activities may change at any time with or without notice.Education, Credentials & Experience RequirementsCurrent GA BSN LicenseMinimum of 1 year management/supervisory/ and education experience preferably in a medical setting.Ability to work with physicians in a collaborative manner.Ability to collaborate with organization and community leaders and organizers to assess healthcare needs of the community and contribute to the Community Needs Assessment.Knowledge of Medicare, Medicaid, third party payors.Active American Heart Association BLS certification within 30 days of hire.
