Position: Residential Case ManagerClassification: Full-time, ExemptReports to: Program ManagerCompany: DePelchin Children's CenterLocation: LaPorte, TXPosition Description:The Residential Case Manager will provide a range of case management services to youth and their families, enabling them to deal with identified problems interpersonally and within relationships with others. The Residential Case Manager role is to optimize the individual strengths of youth, families, and those associated with them in the most effective and efficient manner possible. The Residential Case Manager will provide individual guidance and planning specific to the youth’s needs and care as indicated by the treatment plan.Primary Responsibilities: Collaborate with TDFPS and residential staff to enhance relationships and engagement with clients in the program or assist with preparation for independent livingDevelop, review, and update service plans for respective child in careMaintain regular contact with approved family members, managing conservator and supportive adults to provide updates on the client’s progress while in care, with the goal of developing a partnership between Residential Services and the client or all supportive members. Coordinate and schedule client visits with individuals on a client’s approved contact list. Assist with Residential Services intake procedures and discharge planning for all clients. Responsible for modeling effective communication styles and assistance in advocating for and gaining access to community services, which support clients personal and program goalsResponsible for reporting any changes in the client’s physical condition, environment and/or problems related to the client in a timely manner. Ensure client receives routine care as needed as well as required medical and dental visits in a timely mannerWork closely with program staff as well as other DCC programs and community resources to plan, schedule, and implement services and/or enrichment activitiesFollow all agency/department/program policies and procedures and comply with the legal and regulatory requirements of the Council on Accreditation, Youth for Tomorrow, Residential Child Care Licensing, Residential Child Care Contracts, and other appropriate regulatory bodiesServe as liaison to TDFPS caseworkers and Residential Child Care Licensing as well as to youth when in schoolProvide TDFPS caseworkers and court appointed advocates (e.g., CASA, attorneys) with regular updates on clients’ progress and plan of careCoordinate treatment and participate in permanency planning efforts by consulting and collaborating with TDFPS and invested parties and participating in permanency hearings, reviews and select staffing, when requested. Complete quarterly peer reviews for all client files in the residential program. Assist in treatment planning by participating in monthly treatment team staffing as a member of the Residential Services Treatment Team. Evaluate, re-evaluate and approve restrictions imposed on a youth in the program that has not been reviewed and approved by the service planning team. Required Qualifications:Bachelors degree in health and human services or related field requiredThree (3) years' experience in case management, crisis intervention, and child welfareExperience working with at-risk youthCPR and First Aid Certification (Must be obtained within 60 days of hire as a condition of employment)Preferred Qualifications:Experience working in a therapeutic residential settingKnowledge, Skills, and Abilities:Knowledge of residential service’s safety practices and protocols for children and adolescentsFlexibility and resourcefulness in dealing with new problems and changing circumstancesAbility to work with culturally diverse populations and work forceAbility to work flexible hours which vary according to client needsMust have knowledge of trauma informed care and the practice of crisis interventionAbility to participate effectively in client and case staffing with effective communication skillsMust be able to relate to professionals and families from a variety of different disciplines and agenciesMust have above average assessment, written and verbal communication skillsWork Conditions:Environment: Residential Range of Schedule: Flexible work hours Mon – Fri, 7:00 am – 8:00 pmTravel: Occasional.
