You will help patients become independent by using therapeutic procedures to administer manual exercises, instructing, encouraging and assisting patients with non-manual exercises, ambulatory functional activities and daily living activitiesResponsibilitiesReport significant changes in the patient’s condition (physical or emotional), safety concerns, unusual occurrences, and adverse effects of medical devices to the physical therapist or nurse manager promptlyUtilize physical agents, including but not limited to heat, cold, water, light, electricity, ultrasound and massage as directed on plan of careObserve, record and report to the Physical Therapist, Clinical Manager and Health Care Provider, the patient’s treatment and respond on a timely basisDirect and aid patient/CG in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training Provide and document services in accordance with applicable State, Federal, Medicare, Medical regulations and standards and third-party payer guidelinesWho We’re Looking ForYou are passionate about restoring function, relieving pain, and preventing disability by engaging with patients in the most meaningful setting – the home.

You are experienced and ready to bring your skills and knowledge to patients who need it most. You will provide the very best care in rehabilitation and skilled treatment for patients with chronic conditions, illnesses, surgery or injuries to achieve positive outcomes, improve the quality of life, and reduce risk for hospitalization while delivering exceptional care.

QualificationsCurrent license as a Physical Therapist Assistant in this stateAt least one year of work experience under the supervision of a licensed physical therapistMust have a vehicle, valid current driver’s license and automobile insuranceAbility to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively and submit in a timely manner.
