SummaryThe Memory Connect Wellness Coordinator (MCWC) plays a key role in delivering supportive services to senior living community residents while strengthening engagement with the Senior Doc Primary Care and GUIDE Dementia Support Programs.
Assigned to a pod of partner communities, the MCWC fosters meaningful relationships with residents, families, and staff, serving as a vital bridge between the care team and the community. This role serves as a key point of contact for patients with applicable conditions, working closely with healthcare providers to optimize their care, enhance patient outcomes, and improve overall quality of life. The Memory Connect Wellness Coordinator plays a vital role in coordinating and implementing personalized care plans, providing ongoing support, and facilitating effective communication between patients, caregivers, providers, and other healthcare professionals.
Positional responsibilities include, but are not limited to: Support dementia care activities through the GUIDE programProvide in-person support for telehealth provider visitsAssist with care coordination, care plan updates, and patient enrollmentSupport in the care and coordination and communication of services for residentsProvide companionship to community residentsParticipate in community and company eventsProvide event, marketing, and administrative support across community-based and company initiativesAdditional responsibilities, as assignedRequirements:
