Or wanting to explore the rehabilitation settings for a change.The Fens provides assessment and intervention for men with severe and enduring mental ill-health in a locked environment to support their ongoing recovery and social integration. The whole service is focused on working towards independence and recovery to enable service users to regain health and mental wellness in order to live well in the community.The Fens are seeking to recruit a dynamic Staff Nurse to join their developing team in their 15-bed male high dependency rehabilitation unit in Discovery House. Applications are invited from highly motivated and dynamic candidates with a ‘can do’ attitude to deliver the Trust Vision and Values and help to improve our patient experience within rehabilitation services. This position will offer the successful candidate an exciting and challenging opportunity to work as part of a supportive team, focusing on delivering high standards of care through evidence-based practice.The successful candidate will be responsible for supporting the day-to-day management of the ward, monitoring and auditing, and ensuring the nursing team delivers high-quality care to patients who access our service. Taking up named nurse responsibilities for patients during their time on the Fens, working collaboratively with members of the MDT and outside agencies like MOJ and Probation. In addition to this, the candidate will be able to develop and create a culture that aspires to delivering best practice for individuals using our services and demonstrates effective communication and promotes a culture of transparency and equity within a diverse team. The successful candidate will be responsible for named nurse duties, working closely with their named patient and supporting them through to discharge.Supervisions of Senior Health Care Support WorkersCompleting various MDT reportsAttending MDT meetings such as CPA, Hospital managers and TribunalWorking closely with many MDT professionals, identifying areas of further support and completing referralsHigh-quality documentationAttending your own management supervision and clinical supervisionLincolnshire Partnership NHS Foundation Trust provides mental health services and a number of learning disability, autism and social care services in the county of Lincolnshire.
