DescriptionPosition Title: Transition In Place (TIP) Case ManagerExemption: Non-ExemptLocation: Haverhill and surrounding Merrimack Valley communitiesAnnual Salary: $50,000-$55,000Job SummaryThe Transition in Place (TIP) Program is a U.S. Department of Veterans Affairs Grant and Per Diem (GPD) Program that places Veterans experiencing homelessness into apartments and works with Veterans to assume the lease within six to twelve months.This role will provide case management and supportive mental health and substance use counseling to a caseload of up to 15 Veterans. This role assists Veterans in their transition from homelessness to housing through permanent housing lease assumption. This position employs a variety of evidence-based interventions including Motivational Interviewing, Relapse Prevention and Trauma Informed Care to provide client centered services to Veterans.Job ResponsibilitiesClinical Case Management Maintain a caseload of up to 16 Veterans. Ensure Veterans are swiftly and proactively connected with the services that will best meet their needs and goals. Conduct home visits to help the Veteran gain independent living skills to support their transition to permanent housing. Provide supportive counseling utilizing evidence-based interventions such as: Motivational Interviewing, Relapse Prevention and Trauma Informed Care. Work in a collaborative manner with Veterans towards the goals of obtaining permanent housing through the assumption of the lease by increasing income and benefits and developing self-determination and housing stabilization skills. Work with each Veteran to ensure that they develop a realistic plan and budget to assume the lease within six to twelve months. Work with them to account for all expenses (e.g. rent, utilities, food, transportation, other bills, etc.) to help them understand the financial resources and commitment needed to assume the lease. Complete biopsychosocial assessments, individualized service plans and discharge plans in conjunction with each Veteran, which best meets individual needs and program requirements. Work with the TIP Program Manager to manage crisis situations and intervene with Veterans as needed. Work with the TIP Program Manager to collect program fees. Provide community-based referrals to meet the needs of each Veteran during discharge planning to help maintain independent living. Provide appointment accompaniment as needed. Ensure all data collection and documentation procedures are adhered to according to the deadlines. Work in a collaborative manner with the VA and NECHV Human Services staff to provide coordinated services. Attend and actively participate in department, case conferencing, trainings and program meetings as required. Perform other duties as assigned. Qualifications Bachelor’s Degree, preferred. Two to three years’ experience in case management and working with substance use disorders, mental illness and with a diverse population. Knowledge and commitment to evidence-based models such as Motivational Interviewing, Relapse Prevention and Trauma Informed Care. Ability to employ strong judgement while setting firm boundaries. Crisis intervention skills. Strong communication and interpersonal skills. Essential Functions Massachusetts Driver’s License and ability to travel throughout Merrimack Valley and to the Boston office, as needed, required. Comfort conducting home visits. Ability to navigate services and resources in the Merrimack Valley and surrounding communities. Ability to work comfortably with a multicultural, homeless population experiencing complex challenges. Strong computer and electronic recording keeping skills. Ability to maintain professional and personal boundaries. Ability to write clear progress notes and integrated services plans. Strong written and oral communication skills. Ability to work collaboratively with other Center programs and departments.
