Transition Pathways Officers play an integral role in supporting students who disengage from school in the post-compulsory phase of learning, As a Transition Pathways Officer for the Metropolitan North Region you will work with State High Schools and Secondary Colleges within the region to provide a broad range of services to school staff, students and families to ensure that students who leave school before the completion of Year 12 make a positive transition to a meaningful post-school pathway in further education, training and/or employment.This position will be based at Metropolitan North Region Nundah and/or North Lakes Offices.About The Department Of EducationWorking for the Queensland Department of Education means joining an organisation that values its people and promotes leadership and innovation. Find out more about working with us .Why this Role?As the Transition Pathways Officer, you will have responsibility for the following:Support schools to interpret, develop, refine, and enact departmental policies, practices and procedures relating to students exiting school prior to the completion of Year 12 to ensure they remain in an eligible option.Provide support to schools that will assist in the development and implementation of successful retention and transition strategies and programs that will improve the transition outcomes for ‘at risk' students.Provide appropriate support through the identification of student needs, options and intervention to young people and their families to assist in the transition of young people into viable education, training or employment.Collate and analyse confidential information, identifying needs and barriers to formulate the basis for the development of a school to post-school transition plan for young people (post compulsory participation phase) who leave school before the completion of Year 12 and provide appropriate case management, advocacy and support as required.Forge links with providers of employment services, community agencies, training organisations and employers to create opportunities for young people to engage in programs that support successful transitions from school into employment or training.Please view the Role Description for a full list of responsibilities.Interested?
