Together, we are reimagining how comprehensive, longitudinal care is coordinated and delivered across one of the nation’s leading integrated health systemsWe are seeking a Vice President or Senior Vice President of Ambulatory Care Management to lead enterprise-level care-management strategy, population health integration, and care-model transformation.
