Job Description

Description:•Will the roles be fully remote? Yes•Are there any specific locations the candidates should be in? Texas - DFW, Harris, and Bexar counties•What is the expected schedule (include dates/time/timezone) Monday – Friday, 8am to 5pm CST•What are the day to day job duties?oProvide non-clinical operational support for Waiver coordination.oFacilitate communication between Waiver coordinators and the state Program Support Unit.oResponsible for receiving the initial State referrals for waiver services and working with the Long-Term Services & Supports (LTSS) Coordinator to schedule the initial assessment.oMonitor status for initial assessment, reassessment, and transition assessments for members.oInterface with the State agency that determines eligibility and the LTSS Coordinator to process and obtain approval of Waiver services.oMonitors transition process of Nursing Facility members in custodial beds to return to the community for 'Money Follows the Person' program.oInitiates referral for Medicare and Waiver process if member is not already Medicare or waiver established.oTracks referrals and case documents via designated state systems.oTrack activities occurring within the transitional, assessment and authorization processes, using internal systems and designated state systems, and report results to coordinators, supervisors, and the State agency.oEnsure Medicaid and Waiver eligibility have been requested and received from state partners before transition or services are initiated.oMaintain confidentiality and comply with Health Insurance Portability and Accountability Act (HIPAA).oAttend internal meetings and regularly scheduled calls as assigned.oOther administrative functions as needed.EducationoHigh Diploma/GEDExperience1-3 years’ experience in an administrative support role in healthcare.oExperience demonstrating ability to handle multiple work tasks, prioritize tasks, and solve problems.oExperience demonstrating strong communication skills, internal and external, for the provider outreachoOrganizationoProficient in Microsoft Office with intermediate knowledge in Excel.oAbility to manage a caseload with minimal supervisionoAccurate Documentation/data entry•What IT equipment is required (dual monitors, docking stations, etc.)? laptop, keyboard, mouse, dual monitor, docking station, and headset
