This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model. Please note, this position will require in-person home visits with vulnerable community members.While we thank all interested applicants, we are only able to contact those who we are inviting to an interview.Key Responsibilities: Complete initial home-based physiotherapy assessments within 24 hours of referral Provide follow-up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progression Develop, implement, and adjust individualized treatment plans in alignment with client goals Educate clients and caregivers on home exercise programs, safety strategies, functional mobility, and equipment use Identify and address fall risks, environmental barriers, and safety issues in the home Collaborate with OTs, PSWs, nurses, physicians, and HCCSS/OH@H care coordinators to support coordinated care Complete documentation and charting accurately and in a timely manner according to community-care standards Maintain ongoing communication with the office regarding availability, scheduling updates, and urgent visit capacity Uphold The Care Company's commitment to safe, ethical, and compassionate community careRequirementsScheduling: Flexible part-time schedule (Monday-Friday)Occasional Saturday/Sunday availability based on client needsMust be able to complete an initial in-person home assessment within 24 hours of receiving a visit requestClients are located across Toronto and the Greater Toronto Area, including but not limited to: Downtown Toronto, Midtown, East/West Toronto North York, Richmond Hill, Markham Mississauga, Brampton, Vaughan, Woodbridge Pickering, Ajax, Whitby, Oshawa, Bowmanville and select areas of Durham region Ability to independently manage scheduling, respond promptly to referrals, and coordinate visit times directly with clientsTravel May Be Required Between Client HomesQualification and Experience Requirements: Home and community care experience is a MUST. Applicants without community experience will not be considered. Minimum 2 years of physiotherapy experience in home and community care, preferably with HCCSS/OH@H clients Registered and in good standing with the College of Physiotherapists of Ontario Demonstrated experience in mobility assessments, gait training, falls management, equipment recommendations, and post-discharge rehab Comfortable working independently in the community and making sound clinical decisions Strong communication and documentation skills Ability to travel throughout Toronto and the GTA for client visits Valid driver's license and reliable transportation OR ability to navigate the GTA efficiently via transitBenefitsCompensation: $60-$70 per visit, depending on experience and travel requirements Other Benefits:Flexible part-time scheduling to support work-life balance Opportunity to build a caseload in the areas you prefer within Toronto & GTA Supportive administrative and clinical leadership team Work in a culture that values autonomy, clinical excellence, and client-centered care Opportunities for clinical growth and expanded practice as our programs grow Meaningful, relationship-based care delivered in the comfort of clients' homes
