Community Support Staff (CSS)Baxley Counseling CenterSummary of the Job:Provide Community Support services as defined by DBHDD and Medicaid. Participate in the treatment team planning by helping to create specific, measurable client goals. Teaching the treatment goals in a home based, community setting. The Community Support Staff should be committed to Sanctuary practice utilizing tools that reinforce our shared language to create a trauma-responsive culture.Primary Responsibilities: Has knowledge and aware of formulated Individual Resilience Plan (IRP) and will have access to psychiatric notes, and protected health information located in the clients chart. Provide assistance to client/family/caregiver in facilitation of the IRP, including providing skills support. Share information with the treatment team and is considered a part of the team. Will continuously educate the client about their goals, objectives, action strategies and interventions. Services provided focus on rehabilitative, environmental support and resourcescoordination that promote resiliency and support emotional and functional growth and development. See a client according to assigned and approved units of service. Stay updated on all current Medicaid, licensing, contracting and accrediting requirements. Participate in local inter-agency meetings as needed. Check for completion of case notes, signatures and available units. Utilize personal safety plan for managing emotions and interactions with staff and clients. Complete billing information on services within two days of service provision through the agency’s designated information management and billing system. Provide the number of billable hours assigned to the position; 25 hours weekly for full time staff. Ensure documentation meets requirements of Medicaid, DBHDD, and other CMO audits. See clients according to counseling center schedule.Minimum Qualifications: At least 21 years of age. Bachelor’s degree (from accredited college) with a major in the field of human services, related experience is preferred. Possess the ability, flexibility and maturity to render the duties and responsibilities required of this position. Problem solving ability; able to follow and implement procedures. Strong written and verbal communication skills. Proficient in agency required computer skills. Time management and organizational skills. Adequate training or experience to demonstrate the possessions of knowledge and skills necessary to perform the duties and tasks assigned to this position. Handle multiple tasks simultaneously.
