This pivotal role will direct the Care Coordination & Scheduling team, oversee the development and implementation of client’s plan of care, oversee the front line care staff, and ensure excellent customer service.Position OverviewJoin Golden Harmony as our Director of Care Coordination—a leadership role at the heart of our home care operations! As a care coordination director and senior care operations leader, you will oversee our Care Coordinators, Schedulers, and Nurse Supervisors, ensuring client care plans are expertly executed by our caregiving teams. Your efforts will fuel our commitment to delivering premier in-home care and growing our client base through outstanding service and increased billable hours. The role is full-time (40+ hours/week) with scheduled participation in our after-hours on-call program.Key Responsibilities Lead and supervise the Care Coordination team, including Care Coordinators, Schedulers, and Nurse Supervisors. Supervise care professionals who are working directly with client. Manage and monitor client satisfaction, maintaining high Net Promoter Scores (NPS). Provide input for staff training programs that focus on topics related to senior care. Provide regular feedback and hold team members accountable to performance standards and company goals. Maintain accurate and up-to-date client records, care plans, and scheduling. Direct staffing and scheduling to ensure client care continuity and consistency. Ensure compliance with Department of Health and Human Services regulations. Conduct client visits and follow-up calls to ensure quality of care. Participate in and oversee on-call scheduling rotation. Analyze and prepare reports for weekly leadership meetings to track key operational metrics. Participate in the development of strategies to meet or exceed billable hour targets. Collaborate with the management team to address challenges and improve processes. Direct, interview, hire, and onboard Care Coordinators, Schedulers, Nurse Supervisors. Partner with the community relations team to schedule new client consultations and assessments.Qualifications And Requirements Bachelor’s degree in management, healthcare management, business operations, social work, or nursing preferred. 2-3 years of experience managing teams, preferably in senior living, healthcare or home care services. Proven ability to set expectations, hold teams accountable, and achieve organizational goals. Experience using online scheduling software. Track record of relationship-building to drive business and client engagement. Ability to work “on call” for urgent issues outside of standard business hours. Valid driver’s license and reliable transportation with ability to travel within assigned territory.Essential Skills And Attributes Strong leadership, interpersonal, and motivational skills. Proficient in Microsoft Office (Word, Excel, PowerPoint), Gmail, and Google Workspace applications. Highly organized with the ability to manage multiple projects and shifting priorities. Knowledge of in-home care regulations, industry trends, and best practices. Technologically savvy with the ability to quickly learn new systems and tools. Able to work in diverse environments, handle confidential information, and maintain professional composure under pressure.Work Environment & Physical DemandsThis role involves office work, local travel, and occasional lifting of up to 50 lbs. Position requires corrected vision, hearing within normal range, and frequent use of a computer and telephone.
