Job RoleHealth Care Support Worker Band 3 (£24,937 - £26,598)Permanent37.5 per weeksResponsibilitiesAn exciting opportunity has arisen for a motivated and enthusiastic Band 3 Health Care Support Worker to work in the Locala Community Cardiac Rehabilitation Team in Huddersfield.The post holder will be required to work within the service, supporting excellent holistic care to patients receiving cardiac rehabilitation. This will involve working with the multi-disciplinary team of nurses, physiotherapists, and exercise professionals in a wide variety of community settings. You will be part of the team running the Cardiac Rehabilitation programmes, including setting up and supporting patients in exercise classes, education sessions, assessments and home programme delivery as well as working in the nurse led clinics taking patient observations and histories. There will be some administrative duties to undertake on behalf of the team.We are looking for an individual with multi-disciplinary skills who is confident working as part of a team in a variety of settings.
