Seize this opportunity to be more than just an employee—become an essential part of a movement that truly matters! Job Summary: The Community-Based Patient Navigator will serve as the primary point of contact for patients and caregivers in Atlanta, providing education, guidance, and support in-person or virtually. This role will help patients overcome barriers to care and connect them with resources and services to improve their cancer journey.Key Duties & Responsibilities: Provide individualized support to patients via in-person meetings, phone, email, and online platforms.Educate patients about prostate cancer, treatment options, and available resources.Complete distress screening with patients to determine their current needs, triage patients to appropriate resources and ensure follow-up.Provide emotional support to patients, leading with empathy and compassion.Lead patient and/or caregiver support groups.Maintain accurate documentation of all patient interactions.Identify trends and share insights with the navigation team to inform programming.Other duties as assignedQualifications3 – 5 years of experience in patient navigation, health education, or social services.Knowledge of healthcare systems, insurance, and community resources.Experience in patient education, care coordination, or guiding individuals/families through complex healthcare systems.Strong documentation and data entry skills, with experience using CRM or case management systems.Ability to manage multiple cases effectively.Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse populations. Preferred QualificationsDegree in Social Work, Nursing, Radiation Therapy, Public Health, or a related healthcare field.Familiarity with prostate cancer, men’s health, or chronic disease patient populations.Clinical background with direct patient interaction experience (e.g., social work, nursing, radiation therapy, patient advocacy, or similar).Strong understanding of the cancer care continuum, with emphasis on prostate cancer treatment and survivorship support.Experience in patient education, care coordination, or guiding individuals/families through complex healthcare systems.Based in Atlanta, GeorgiaFamiliarity with community resources, support services, and healthcare navigation best practices.Certification or training in patient navigation, such as:Oncology Patient Navigator–Certified Generalist (OPN-CG, AONN+)National Certified Patient Navigator (NCPN)Board Certified Patient Advocate (BCPA)Community Health Worker (CHW) Certification (state-recognized, if applicable)Completion of American Cancer Society navigation training or participation in the ACS LION programPhysical RequirementsConstant use of a computer and other office productivity equipment, such as a computer printer, calculator, and copier.Prolonged periods of a stationary position at least 50% of the time.The ability to recognize details at close range for extended periods of time.The ability to communicate information and ideas so others will understand. The position requires approximately 70% local travel within the city to visit patients and meet with healthcare partners (travel may vary based on program and community needs).
