Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs.We are seeking a Mental Health Counselor for the Crotona Park residence which is The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. The majority of Crotona Park residents will be referred by the New York City HIV/AIDS Services Administration (HASA) and some by the New York City Housing Authority (NYCHA).The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.We offer a competitive salary and a comprehensive benefits package, including medical and dental coverage, paid retirement plan participation, paid sick, personal and vacation time, paid holidays, discounted gym memberships, and more.Key Responsibilities:Maintain a caseload and provide regular case management services to these individualsMaintain resident records in compliance with agency and funding standardsConduct and review assessmentsDevelop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency Monitor and ensure residents are linked to healthcare providersProvide counseling and crisis intervention as necessaryAssist residents in securing and maintaining benefitsMaintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programsFacilitate groups and activities that promote health and independent living skills developmentMonitor and assist residents with rent payments and budgetingConduct regular unit inspectionsEducate residents about approaches to mental health Compile resources on mental health information and treatment strategiesAttend ongoing training to enhance the services provided to the residentsAssist with completion of HASA’s weekly and monthly reports as well as any other relevant reportsOther duties as assignedThe Ideal Candidate:You have a Master’s Degree in Social Work or a related field and a LCSW.
