Pharmacist Prescriber - Tend Pakuranga

Work as part of our clinical team at Tend Pakuranga!The Pharmacist Prescriber plays an integral role as part of the wider care team model at Tend, working at top of scope to ensure effective and efficient delivery of pharmacist services to patients (including prescribing) and providing support to GPs, nurses and the wider Tend team.You will also provide care directly to people and their whānau, and support the care team to provide accessible, equitable, and optimal use and management of medication treatments.This is a fixed-term full-time position until July 2026 due to external funding.As part of your role, you will be responsible for:Medicine management and optimisationMedication reconciliationAdvice, education and supportManagement of long-term conditionsQuality, Training and RiskEquitable & culturally safe care.
