An exciting opportunity to join the NHS Rapid Response Torbay team has arisen. The service provides support to patients at home following an acute medical crisis, end of life support or support at home following a hospital discharge.We are looking for a Band 4 Senior Referral Coordinator, working on a shift pattern, to join our team and this front-line service. The service operates 7 days a week including all bank holidays.The post holder will be responsible for the Referral and Coordination on behalf of the Rapid Response ServicesYou will need to be able to co-ordinate detailed referrals from Health and Social Care Professionals, communicate and answer queries from all disciplines including Care Providers, members of the public and users of the service, and to provide direct line management support and supervision of the support workers, ensuring strict compliance with Trust policies and procedures.The service operates on a 7 day rota system, including weekends and Bank Holidays. The post will be based at St Edmunds in Torbay although you could be asked to work throughout the Torbay & South Devon area.Torbay and South Devon NHS Foundation Trust is a unique organisation providing acute health care services from Torbay Hospital, community health services and adult social care.We have around 500,000 face-to-face contacts with patients in their homes and communities each year and see over 78,000 people in our A&E department annually.
