Our goal is to reduce hospital readmissions and empower seniors to age in place with dignity, comfort, and continuity of care.As a Staff Nurse, you’ll play a key role in bringing medical support into the homes of vulnerable clients, helping them maintain quality of life in the community they know and love.Key ResponsibilitiesConduct regular home visits and comprehensive nursing assessmentsPerform nursing procedures such as wound care, catheterisation, NG tube insertion/removal, etc.Work closely with doctors and caregivers to coordinate care plansEducate clients and families on basic home nursing and self-care techniquesRefer clients to relevant financial or psychosocial support servicesMaintain accurate, timely nursing documentation in line with PDPA standardsAchieve a minimum of 7 home visits per daySupport occasional outreach or fundraising activities as neededWhat You BringDiploma or Degree in Nursing, registered with SNB1–2 years of clinical experience (community care experience a plus)Able to work independently and travel for daily home visitsStrong communication and interpersonal skillsCompassionate, reliable, and adaptable to diverse home environmentsWhy Join Us?Make a real difference by bringing care into homes and lives.
