Successful applicants will receive all recruitment related correspondence via the email account registered on the application form.Aneurin Bevan University Health Board support flexible working.Please note that this vacancy may be withdrawn at any time should it be filled via the internal redeployment processJob OverviewAn exciting opportunity has arisen for a Band 4 Assistant Practitioner to join the Bargoed District Nursing Team on a permanent contract for 18.5 hours, which would include some weekend working.The Bargoed team provide clinical care and interventions for patients in their own home under the medical governance of Primary Care GP's.The District Nursing/Community team in Bargoed works as a partner within a wider multi-disciplinary team which includes CRT Frailty Consultant, General Practitioners, Care at home, Physiotherapists, Occupational Therapists, Social Workers, Support and Wellbeing workers and the Voluntary Sector.An informal visit by anyone prior to application would be welcomed by the team.Main duties of the jobAre you motivated, enthusiastic and want to progress in your career?An exciting opportunity has arisen for a suitably qualified Band 4 Assistant Practitioner to join the Bargoed community team in the provision of clinical care to individuals in their own home.As Assistant Practitioner you will work with the wider community MDT under the delegation of your Band 7, Band 6 and a team of Registered Nurses. You will utilise and develop a wide variety of skills to meet the emotional, physiological, and clinical needs of individuals.The post holder will work collaboratively and support members of the multidisciplinary team in the delivery, development and management of health and appropriate referral for social care issues.
