Location: 3864 N 27th Ave, Phoenix, AZ 85017Full-time ScheduleFlexible Scheduling40 Hours Per WeekPosition Summary: The Behavioral Health Technician (BHT) for Bridging the Gap – Re-Entry program provides direct care services: intakes, assessments, and coordinates linkages into necessary community services and connections to providers. The BHT coordinates member referrals, provides case management services as needed, completes tasks that require serving the members who are enrolled in the Bridging the Gap Re-Entry /TIP Justice program; liaises with other Program staff, Program Supervisor, Director and medical records staff. The BHT delivers health and wellness and recovery-oriented services to members. The BHT participates in vital program evaluation procedures and is responsible for accurate and professional record keeping in required database platforms. This position reports to the BTG Reentry Program Supervisor.Essential DutiesResponsible for direct face to face community-based services to assist client’s re-entering into the community from jail and prison. Ensures linkage into community services and resourcesOffers in-reach into jail, prison, sober living housing and treatment programs for the purpose of engaging member into BTG Re-Entry / TIP services. Orients potential clients to the BTG Re-Entry/TIP program.Consistently meets Service Delivery expectations including $2150 weekly productivity, Compliance, and timely filing (notes completed by midnight same day).Provides transportation for members as appropriate and as medically necessary, and determined by the Re-Entry/ TIP team, in personal vehicle or in Terros Health provided vehicle, when available.Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate. Coordinates care with a member’s family, behavioral and general medical providers, community resources, and other involved supports.
