Schedule: Sunday-Thursday 2:00pm-10:30pmGeneral Description Of The PositionThe Case Manager at Tatum Psychology Employment Group is responsible for overseeing documentation of patient’s intake, progress, and discharge, as well as liking them with appropriate social, occupational, and educational resources as needed. The Case Manager works well with the treatment team to plan and coordinate the client’s progress through the continuum of care.Essential Functions Patient Admission & OrientationAdmits new patients by completing the intake process and new patient orientation. Assessment & Treatment PlanningEvaluates patients’ needs for medical, psycho-social, and psychiatric treatment. Case Management & Patient MonitoringMonitors cases by verifying patients’ attendance, observing, and evaluating progress in treatment; advocating for needed services; obtaining additional resources; providing personal support.Maintain patients’ records by reviewing case notes and documenting progress. Communication & Interdisciplinary CollaborationCommunicates patients’ progress by participating in weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences. Discharge Planning & Post-Care CoordinationPrepares patients’ discharge by creating discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics. Staff Development & TrainingImproves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.Additional FunctionsPerforms other duties as may be assigned.Follows and supports TPEG policies and procedures.Works collaboratively and cooperatively with internal and external partners.Represents TPEG, Inc., by embodying the mission, vision and values of the organization.Travels occasionally during the workday and on occasional overnight stay. Employees who hold this position may be asked to perform other duties as assigned.RequirementsMINIMUM QUALIFICATIONS:EDUCATION: Bachelor's Degree in Psychology, Social Work, or related field.RELATED EXPERIENCE: Two (1) years’ experience in a behavioral health care setting.SKILLS: Strong leadership, communication, and organizational skills. Ability to work independently and as part of a team.OTHER EXPERIENCE / SKILLS REQUIRED:Experience working with culturally diverse individuals and communities or have otherwise demonstrated a commitment to strengthening engagement of a diverse community and skill in communicating with diverse populations.Familiarity with equity and/or diversity initiatives within an organization.Written and oral communication skills sufficient to perform essential functions.Demonstrated ability to make decisions independently and use initiative to accomplish complex assignments with general instruction and guidance.Demonstrated ability to exercise judgment and integrity on the job with confidentiality, tact, and diplomacy.Demonstrated ability to organize time and other resources to perform multiple tasks.Demonstrated ability to complete work accurately and in a timely manner with attention to detail.Demonstrated ability to work well with others and to provide effective team leadership.Proficiency in word processing and database and/or spreadsheet applications.Physical and mental attributes sufficient to perform essential functions.Valid Driver's License / Clean Driving RecordAbility to pass a Department of Justice (DOJ) Background ClearancePreferred QualificationsExperience in dual-diagnosis or mental health treatment preferred.Proficient in English LanguageProficient communicatorWorking ConditionsWork settings vary from offices, program sites, and stakeholder locationsTravel modes can include the use of company or personal transportationSalary Description $20-$24 / HR
