Day-to-day tasks include managing patient cases, coordinating care plans, providing support for home care requests, and addressing patient needs with excellent customer service. The individual in this role will play a critical part in ensuring that patients receive holistic and well-organized care.QualificationsProficiency in Care Coordination and Case Management to ensure effective organization and follow-through on patient care.Strong Communication skills for liaising with patients, families, and healthcare providers efficiently.Experience in Home Health services to effectively address patient needs outside of a hospital setting.Background in Customer Service to deliver compassionate and professional support to diverse stakeholders.Ability to work independently in a remote environment.Organizational and problem-solving skills are highly desirable.Experience in healthcare or medical services is an advantage. Bachelor's degree in healthcare administration, nursing, or a related field preferred.
