Job Description

The Recovery Program Administrator position is contingent upon grantfunds, employment will depend on the successful acquisition andcontinuation of the behavioral health grant. The Recovery ProgramAdministrator ensures effective coordination, communication, andfollow-through across the CHP Recovery Team and CHP sites. This positioncombines patient care coordination, project administration, datatracking and input, and quality improvement to strengthen programdelivery and outcomes. The ideal candidate is highly organized,mission-driven, and able to manage their time and balance multiplepriorities in a fast-paced environment.## Position RequirementsEssential Skills & Qualifications Program Coordination** - Manageadministrative and coordination functions that enhance case management,resource linkage, and patient engagement. - Coordinate workflows withinthe CHP Recovery Team (primary care, counseling, psychiatry, MAT,recovery, and crisis intervention) and with outside agencies to reducebarriers to care. - Track outcomes and identify gaps to support qualityimprovement in the program. - Liaise between the CHP Recovery Team andthe ACO case management team around housing and critical needssupport. - Collaborate with internal staff and community partners toimprove patient access to essential services. - Foster teamwork, sharedpurpose, and coordinated patient care within the recovery program.Patient Care Coordination** - Coordinate patient visits and schedulingfor medication for addiction treatment (MAT). - Monitor controlledsubstance prescriptions and documentation requirements. - Ensure MATtreatment agreements and controlled substance agreements are completedand filed appropriately. - Track injectable medication orders fromparticipating pharmacies. - Maintain compliance with all documentationand follow up protocols. **Case Management and Care Coordination** -Manage releases of information (ROIs) and external referrals forbehavioral health and substance use disorder services. - Communicatewith outside partners to support smooth transitions of care and timelyinformation exchange. - Follow up on patient needs related to housing,insurance, or social determinants of health, in coordination withACO/CHW case management and clinic management. **Group Facilitation andPatient Engagement** - Organize and support group sessions on recovery,mental health, and wellness topics. - Conduct outreach, recruitment, andattendance follow-up for group programs. - Track participation andoutcomes of group interventions. **Data and Quality Management** - Trainin the federal RedCap portal for NOMS data, track completion of dataentry, and assist with entry. - Oversee quarterly and six-monthreassessments required by the program grant. - Collect and monitor datarelated to patient engagement, outcomes, and quality. **Experience**(Preferred but not required) - At least 3 years\' experience inoutpatient health care (primary care, behavioral health, or substanceuse treatment) - Demonstrated success in case management, carecoordination, or healthcare project work. - Strong communication,organization, and time management skills - Proficiency in EMR systemsand office software - Team-oriented with proven leadership andcollaboration skills - Ability to engage patients with empathy, culturalhumility, and trauma-informed care principles - Familiarity withharm-reduction and recovery-oriented approaches - Bilingual(English/Spanish) **Education and Training:** Associate degree orhigher, or equivalent work experience [To view the full desciption andapply, please clickhere.](https://recruiting.myapps.paychex.com/appone/MainInfoReq.asp?R_ID=6997713){target="blank"rel="noopener noreferrer"}
