As part of our team, you will have the unique opportunity to help promote independence and empower individuals to achieve their personal goals.The Community Resource Team is a collaborative service between Aneurin Bevan University Health Board and Blaenau Gwent Social Services, dedicated to providing individuals with the right intervention at the right time, from the right professional(s). As a Team Lead Occupational Therapist, you will have the chance to make a real difference in the lives of adults in need of support to remain in their own homes. You will be an integral part of the wider multi-disciplinary team comprising of Physiotherapists, Occupational Therapists, Health and Well Being Support Workers, Social Workers, Medics and Nursing staff. You will work alongside other services such as the Rapid Response Team, Community Hospitals, Acute Hospitals, Home Care and Care Agencies.Main duties of the jobThe Community Resources Team works with individuals to enable them to be supported in their own home wherever possible, by preventing hospital admissions, facilitating the timely discharge of service users from acute and community hospitals and assessing for potential long term care and support needs.Working in collaboration with the CRT Team Manager and ABUHB's Occupational Therapy Service Professional Clinical Lead, the post holder will undertake a clinical leadership and operational role, you will lead and influence the ongoing development of OT provision within clinical areas, whilst carrying a varied caseload of people. You must be confident and effective in the delivery of occupation focused, high quality interventions.Occupational Therapy is a highly valued service within Blaenau Gwent, and we are strongly committed to service development, standard setting, clinical effectiveness and lifelong learning.
